Registration Deadline is September 24, 2012

1. Please email Jane Richardson, Tournament Director, at: chaostourney@scfha.com to inform her of your interest to enter the tournament.
    
2. Print out the Registration Form and mail with your cheque, payable to "SCFHA", to:

Your team will not be officially entered into the tournament until your Registration Form AND cheque are received.  You will receive a confirmation email upon receipt of your COMPLETE registration.
